Welcome to the office of Damian Garcia, M.D., Family Medicine! If you are coming for the first time, you can prepare by printing the new patient paperwork and filling it out at home. We need you to fill out and sign the Patient Health Information pages, the Demographics pages, the Office and Payment Policies notice, and the Acknowledgement of Receipt of Notice of Privacy Practices. You may fax the completed paperwork to us as 972-224-8084, or you may bring it with you to your first appointment. Because email is easily hacked, please do not email your private health information to us. EMAILED INVOICES WITH OUR PHONE NUMBER ARE NOT FROM OUR OFFICE.
